2019年北京市朝阳区高中及以下阶段新生入学肺结核筛查结果分析

目的 分析北京市朝阳区高中及以下阶段新生入学肺结核筛查结果，为学校结核病防控工作提供参考依据。 方法 幼儿园、小学、非寄宿制初中新生首先完成肺结核可疑症状问卷调查，有可疑症状者需进行结核菌素皮肤试验（PPD试验），PPD强阳性或未完成PPD试验者再进一步完成X线胸片检查。高中及寄宿制初中新生均需完成问卷调查和PPD试验，有可疑症状或PPD强阳性或未完成PPD试验者再进行X线胸片检查。 结果 2019年北京市朝阳区550所学校完成了新生肺结核筛查。问卷调查70 835人，有结核病可疑症状者109人，可疑症状率1.54‰，高中（9.36‰）高于幼儿园（0.55‰）、小学（0.63‰）、初中（0.69‰）。PPD试验8 567人，强阳性104人，强阳性率为1.21%，高中组（1.33%）高于幼小初中组（0.64%），差异有统计学意义（x2=4.72， P =0.03）。X线胸片检查461人，影像学异常9人（全部为高中学生），其中1名女生诊断为活动性肺结核。 结论 新生入学肺结核筛查工作具有重要意义，应加强高中学生的结核病防控管理工作。

Tuberculosis screening results among new students of high schools and below in Chaoyang district of Beijing, 2019

To analyze the results of tuberculosis( TB) screening among new students of high schools and below in Chaoyang District of Beijing City,and to provide reference for TB prevention and control in schools. Methods The new students of kindergarten,primary school and non-boarding junior middle school completed investigation on suspicious symptoms of TB first,those with suspicious symptoms had to carry out the tuberculin skin test( PPD test),and those with strong positive or incomplete PPD test had to carry out X-ray chest examination. The new students of senior middle school and boarding junior middle school completed investigation on suspicious symptoms of TB and carried out PPD test,and those with suspicious symptoms or strong positive PPD test or incomplete the PPD test had to carry out X-ray examination. Results In2019,550 schools complete TB screening among new students in Chaoyang District of Beijing City. There are 70 835 investigations with 109 suspicious symptoms of TB,the suspicious symptom rate is 1. 54‰,the rate of senior middle school( 9. 36‰) is higher than the kindergarten( 0. 55‰),the primary school( 0. 63‰) and the junior middle school( 0. 69 ‰). There are 8 567 PPD tests with 104 strong positive,the strong positive rate is 1. 21%,the rate of senior middle school group( 1. 33%) is higher than the other group( 0. 64%),the difference is statistically significant( χ2= 4. 72,P = 0. 03). There are461 cases of X-ray examination with 9 cases of imaging abnormality( all of them were senior middle school students),one girl is diagnosed as active tuberculosis. Conclusion The work of TB screening among new students is meaningful. The prevention and control TB should be strengthened in senior middle school.
