巨睾并殖吸虫人体致病的发现及其疫区调查

摘    要:1987~1991年我们对桂北融水县大年乡高汉、中止村作肺吸虫病调查,圆顶华溪蟹肺吸虫囊蚴感染率高达20.5%(100/487),拟钉螺的尾蚴感染率为0.4%(105/26170),居民的肺吸虫抗原皮试及酶联免疫吸附试验阳性率分别为65.7%(448/682)和47.5%(252/530).有胸痛、咳嗽、喀血、咳铁锈色痰等表现者为18.4%(55/299),痰检粪检虫卵阳性率为7.4%(22/297)。根据人工感染家犬所获虫体、虫卵及尾蚴、囊蚴等形态观察、比较,结合患者的临床表现认为当地存在两种肺吸虫病病原,一为斯氏狸殖吸虫,另一为巨睾并殖吸虫。后者的人体感染为首次发现。融水县肺吸虫病流行区的调查结果显示:当地人群感染率之高、危害之严重为文献所少见。

Abstract:From 1987 to 1991, a survey of paragonimiasis was conducted in 2 villages located in the hilly district in north-west part of the Rongshui county of Guangxi. The infection rate of Parag-onimus cyst sinopotamon teritisum rohgienseis was as high as 20. 5% (100/487) and the infection rate of Paragonimus cercaria Tricvla sp. was 0. 4% (105/26170) The positive rates of more than 3-year-old residents with eating crab history, for paragonimiasis antigen, 497 with IDT and 360 with ELISA were 82. 1% and 68.6% respectively. Positive rate of constitutional symptoms of 299 with IDT was 18.4%, the symptoms were thoracic pain, cough with rusty-brown sputum, haemoptysis and the positive rates of detection for egg in sputum and feces were 7. 7%.According to the morphologic observation and comparison on worm , egg recovered from 27 inoculated dogs , cercaria and cyst and clinical analysis on general symptoms of infected patients , the existence of 2 kinds of paragonimus was demonst rated. One of them is P. skrjabini and the other P. macrorchis.The discovery of human P. macTorchis infection and endemic area of paragonimiasis in Rongshui County, Guangxi province was the first time in this country.
