胸段食管癌三维适形治疗与常规放疗肺及脊髓受量比较

目的比较三维适形治疗方案与常规治疗方案的剂量分布，考察三维适形治疗技术在胸段食管癌放疗中对脊髓、肺等器官保护的优势。方法对51例中段食管癌患者分别进行常规方案和全程三维适形方案计划设计。以62Gy为处方剂量，96％剂量线包绕食管肿瘤，剂量分布评价指标为：16cm长度脊髓平均剂量，最大剂量，36Gy脊髓剂量长度，肺平均剂量，肺V20，肺V30。结果全程三维适形方案的脊髓平均受量为13．1Gy，远低于常规方案的23．2Gy；肺平均受量为19．3Gy，与常规方案的19．6Gy相近；肺V20为24．8％，低于常规方案的33．8％；肺V30为5．2％，低于常规方案的8．6％。结论全程三维适形治疗方案具有明显的优势。胸段食管癌的全程三维适形治疗方案在正常组织的保护上明显好于传统的常规方案。

A comsparison of lung dose and spinal cord dose in three dimension conformal radiotherapy (3DCRT) and routine radiotherapy on thoracic esophageal carcinoma
