拇指尺背侧和示指背侧岛状皮瓣修复拇指皮损临床对比研究

目的探讨拇指尺背侧和示指背侧岛状皮瓣修复拇指皮损临床效果差异。方法笔者以2012年5月~2014年5月收治的行拇指皮损修复患者80例作为研究对象,随机分为A组和B组,每组各40例;其中A组采用示指背侧顺行岛状皮瓣修复,B组采用拇指尺背侧逆行岛状皮瓣修复。比较两组患者指腹质地感觉、拇指外观、痛觉及触觉满意率和两点辨别觉水平等。结果 B组指腹质地感觉满意率显著高于A组,差异有统计学意义(P0.05);两组拇指外观、痛觉及触觉满意率比较差异无统计学意义(P0.05);B组两点辨别觉指标值显著多于A组,差异有统计学意义(P0.05)。结论拇指尺背侧皮瓣应用于拇指皮损修复术中创伤小,操作简便,可有效改善指腹质地感觉,提高术后美观性,临床效果优于示指背侧岛状皮瓣。

Clinical comparative study of dorsal flap of thumb and dorsal flap of index finger for the repair of thumb lesions

Objective To investigate the difference of clinical effects of dorsal side of thumb finger flap and dorsal side of index finger flap for the repair of thumb lesions .Methods Eighty patients with thumb lesions from May 2012 to May 2014 in our hospital were chosen and randomly divided into two groups: group A (40 patients) with dorsal flap of index finger and group B ( 40 patients ) with dorsal flap of thumb;and the satisfactory rate of sense,appearance,pain and touch and level of two-point discrimination of the two groups were compared .Results The satisfactory rate of thumb sense of group B was significant higher than group A (P<0.05).There was no sig-nificant difference in satisfactory rate of thumb appearance ,pain and touch between the two groups (P>0.05).The level of two-point discrimination of group B was less than group A with significant difference (P<0.05).Conclu-sion Compared with dorsal flap of index finger ,dorsal flap of thumb finger for the repair of thumb lesions can effi-ciently promote the recovery of sensory function of thumb and improve the postoperative appearance .
