晚期食管癌对铂类药物化疗敏感性与p53和nm23-H1表达的关系

目的：探讨p53和nm23-H1的表达与晚期食管癌化疗敏感性的关系。方法：以免疫组化对57例食管癌患者进行p53和nm23-H1表达的检测观察,以铂类为主化疗2个疗程后进行效果评价。结果：p53的阳性率为54.4%（31/57）,与性别、年龄、淋巴结转移、位置、细胞分化、临床病理分期均无关;nm23-H1阳性率为42.1%（24/57）,与临床病理分期有关,而与性别、年龄、淋巴结转移、位置、细胞分化无关;恶性度越高,nm23-H1表达越高;晚期食管癌中,nm23-H1（＋）的患者化疗敏感性明显优于nm23-H1（-）患者;而p53（-）患者化疗敏感性优于p53（＋）。结论：p53（＋）和nm23-H1（-）可能提示术后晚期食管癌患者对以铂类药物为主的化疗耐药性高。而nm23-H1（＋）和p53（-）可能提示对术后铂类的化疗效果敏感。

Relationship between expressions of p53 and nm23-H1 and response of cisplatinum-based adjuvant chemotherapy to advanced esophagus carcinoma

OBJECTIVE:To investigated the relationship between the expressions of p53 and nm23-H1 and the response of chemotherapy to advanced esophagus carcinoma.METHODS:Fifty-seven patients were allotted.Pathological sections were immunohistochemically stained with antibody specific to p53 and nm23-H1.RESULTS:By the immunohistochemical method,p53 and nm23-H1 expressions were detected in 54.4%(31/57)and 42.1%(24/57)of the pathological sections.nm23-H1 was positively correlated with the tumor stage and p53 was positively correlated with nothing.The effect of chemotherapy of p53 negative group was statistically superior to p53 positive group.nm23-H1 positive group was statistically superior to nm23-H1 negative group.The combination of p53 negative and nm23-H1 positive had the better response to chemotherapy.CONCLUSIONS:p53 negative group and nm23-H1 positive in esophagus carcinoma could predict the sesentivity of cisplatin-based adjuvant chemotherapy effect in esophagus carcinoma patients followed by surgery.
