模拟定位机引导下经皮穿刺集束巨能刀射频治疗肺癌初探

目的探讨局部麻醉和模拟定位机引导在集束巨能刀治疗肺癌中的可行性及射频治疗的适应证、治疗效果、并发症的预防和处理。方法采用质量浓度为0.5%普鲁卡因局部浸润麻醉,模拟定位机引导下经皮穿刺集束巨能刀治疗肺癌25例。结果 CT 显示肿瘤消失5例,空洞形成或直径缩小17例,无变化1例,增大2例。治疗中患者均有不同程度的胸腔内热感、大量出汗及不同程度的疼痛感。并发症包括气胸4例、皮下气肿3例、发热18例、咯血9例、慢性支气管炎急性发作3例,无血胸和局部感染。结论模拟定位机引导下集束巨能刀射频治疗肺癌疗效肯定,特别是对直径<3cm 的周围型肺癌疗效尤为满意,安全性好,定位准确可靠,无严重并发症出现。

Radiofrequency Ablation of Lung Malignancies Using Percutaneous Assembly - Energy Scalpel Guided by Analogue - Localization

Objective To study the feasihility of Assembly-Energy Scalpel in the treatment of lung malignancies guided by local anesthesia and ananlogue-localization,and its indication,therapeutic efficacy,prevention and management of various complica- tions.Methods Twenty-five cases of lung cancer were treated via percutaneous Assembly-Energy Scalpel under local anesthesia and ana- logue-localization guide.Results CT scan shows that tumour disappeared in 5 cases,internal cystic change or diameter-reduction of tumours occured in 17 cases,no change in 1 case and diameter-enlargement in 2 cases.Complicatioas include 4 cases of pneumothorax,3 cases of sub- cutaneous emphysema,18 cases of Fever,9 cases of hemoptysis and 3 cases of acute onset of chornic bronchitis.There's no severe pain, hemothorax and local infection occured.Conclusion The effect of Assembly-Energy Scalpel radiofrequency ablation guided by analogue-local- ization is affirmative in the treatment of lung malignances,especially in the treatment of peripheral lung tumours which diameter is less than 3cm.It's a reliable theraputic method with high security and precise localization.There was no severe complications occurred in the treatment.
