摘    要:目的 了解2015年云南省德宏傣族景颇族自治州(德宏州)未接受抗病毒治疗的HIV感染者的耐药株传播水平。方法 2015年1-7月在德宏州收集72份新报告的16~25岁HIV-1感染者的血浆样品,进行HIV-1基因型耐药检测和耐药株传播水平分析。结果 48份完成了基因型及耐药鉴定,其中中国籍占31.2%(15/48),缅甸籍占68.8%(33/48)。通过进化分析对pol区进行分型,主要的基因型包括URF(52.08%,25/48)、CRF01_AE(16.67%,8/48)、CRF07_BC(10.42%,5/48)、B亚型(6.25%,3/48)、C亚型(6.25%,3/48)、CRF57_BC(6.25%,3/48)和CRF08_BC(2.08%,1/48)。在4个序列中分别检测到1个针对非核苷类反转录酶抑制剂的耐药突变位点(K103N)和2个针对核苷类反转录酶抑制剂的耐药突变位点(T69D和T215I),按照耐药警戒线的统计方法估算耐药株流行率为5%~15%。结论 2015年德宏州16~25岁新报告的HIV-1感染人群中外籍比例较高,HIV-1基因型复杂,HIV-1传播性耐药达中度水平。
