食管癌贲门癌合并糖尿病的围手术期处理

目的探讨食管癌贲门癌合并糖尿病的外科治疗和围手术期处理。方法对41例食管癌贲门癌合并糖尿病患者采用控制血糖结合手术切除肿瘤治疗。结果所有患者手术切除率为100%,术后并发症6例,包括肺部感染2例,吻合口瘘1例,单纯脓胸1例,切口感染2例,均经治疗后痊愈,无手术死亡患者。结论糖尿病引起代谢紊乱易发生各种感染及愈合不良,使术后并发症发生率显著高于同期非糖尿病患者,说明糖尿病围手术期处理的重要性。对食管癌贲门癌合并糖尿病患者的手术治疗,应采取积极的态度,只要重视围手术期的处理,应该是安全的。

Experience of perioperative treatment in the diabetic patient with esophageal cardia cancer

Objective To explore a better method for the operation and complication of esophagus cardiac cancer combined with diabetes. Methods Control blood sugar,combined with operation to remove tumor for 41 cases of oseophagus cardia cancer combined with diabetes. Results All cases had been taken operation,6 had post - operation compllcation,including lung inflammation 2, anastomose fistula 1, chylothorax 1, cut infection 2 cases, all cured after preserving therapy,no was dead. Conclusion Diabetes can cause disordered metabolism,easy for infections and bad cure, make post - operation complication rate markedly higher than that of the same kind of non - diabetes ,therefore, dealing perioperation period of diabetes is very importance. The surgery operation in the diabetic patients with esophageal cancer is safe, if the fasting blood sugar had been controlled strictly during the perioperative period.
