小儿SIRS与简化危重病例评分的临床应用评价

摘    要:目的探讨小儿全身炎症反应综合征(SIRS)和简化小儿危重病例评分法(PCIS)的临床应用价值.方法对2003年9月-2004年9月的1670例住院患儿中符合SIRS诊断标准的320例患儿逐一进行简易PCIS评估,观察两者对病情评估的准确性及相关性.结果随着小儿SIRS诊断符合项目增多,简易PCIS分值减低,危重、极危重组病儿增多,死亡率增大,两者有良好的相关性.结论小儿SIRS和简易PCIS都可用于临床评估病情,SIRS诊断标准过松,两者结合应用能更准确判断病情,值得临床推广应用.

Abstract:Objective The article aims at probing into the clinical application value of infant's SIRS and PCIS by evaluating the 320 infant inpatients in the way of simplified PCIS and observing the accuracy and relevancy of the two with regard to the illness.Methods These 320 infant inpatients were chosen from the 1670 infant inpatients whose syndromes are in accordance with the diagnostic standard of SIRS and they were in hospital from September 2003 to September 2004.Results The result shows: with the increase of the items that are up to the diagnostic standard,the value of simplified PCIS decreases;the number of seriously and extremely seriously infant patients rises up;mortality rate keeps increasing,and there has a good relevancy between the two.Conclusion Thus it can be concluded that both infant's SIRS and simplified PCIS can well be applied to the clinical assessment of illness.The diagnostic standard of SIRS is rather low while a combination of the two can help to make accurate diagnosis of the illness and thus worth of being popularizing in clinical application.
