用二维斑点追踪技术评价正常犬肾交感神经消融后左房变化

摘    要:目的:用超声二维斑点追踪显像技术(STI)检测肾动脉交感神经经导管射频消融后左房的变化。方法:对20只犬的肾交感神经进行射频消融,分别于消融前和消融6周后观察肾交感神经消融犬的血压变化及左房各指标变化。采集犬心尖两腔观、四腔观清晰图像,用Qlab软件分析左房前壁、下壁、间隔及侧壁基底段、中间段及心底段12节段应变率同步性指标,包括:左房储蓄功能期峰值应变率达峰时间及达峰时间标准差,左房主动收缩期峰值应变率达峰时间及达峰时间标准差。分析左房间隔及侧壁基底段消融前后应变率变化。结果:与消融前相比,消融后收缩压和舒张压均无明显变化(P>0.05)。消融后犬R-R间期呈延长趋势,但无统计学差异,消融前后心脏各腔室大小没有明显变化,左房间隔及侧壁基底段各时相应变率无变化,左房同步性无明显变化(P>0.05)。结论:消融正常犬肾交感神经后,血压无影响,左房心肌运动功能无变化,对同步性也无影响,说明肾交感神经消融对心房是安全的。

Evaluation the Left Artrial on Normal Dogs After Transcatheter Renal Sympathetic Denervation by Two-Dimensional Speckle Tracking Imaging
Abstract:Objective:To evaluate left artrial function of normal dogs after transcatheter renal sympathetic denervation(RSD)by two dimensional strain imaging.Methods:Renal sympathetic nerves of 20 dogs were ablated,and the index of heart and blood pressure were detected 6weeks later.Standard 2Dimages were acquired in the 2-and 4-apical views.The time to peak longitudinal strain rate and time to peak contraction strain rate were measured for each segment in the level of the basal segments,middle sections and apical in Qlab software,and the standard deviation was calculated.The strain rate was measured in the septum and lateral of basal segments,and parameters were compared before and after ablation.Results:The systolic pressure and diastolic pressure have no changes after the RSD(P<0.05),and the strain rate of septum and lateral of basalsegment have no significant difference(P>0.05),the synchrony parameters have no significant differences too(P >0.05),and the size of the heart cavity have no differences(P >0.05).Conclusion:The blood pressure of the normal dogs have no changes after the RSD,and the size of the heart cavitythe function of the left atrial myocardial and the synchrony parameters have no significant differences too.This study demonstrated that RSD has no effect on left atrial function.
