摘    要:目的探讨18氟脱氧葡萄糖正电子发射计算机断层扫描(18F-FDG PET)对肺癌诊断、分期、疗效评价及随访等方面的临床意义.方法对56例肺癌患者18F-FDG PET显像的临床资料进行回顾性分析,所有病灶的标准摄取值(SV)均以2.5界定为肺癌病灶.结果PET用于肺癌临床:①敏感性92.8%;②特异性89.3%;③对肺癌诊断的准确率91.O%;④假阳性率5.4%,假阴性率3.6%;⑤对肺癌纵隔淋巴结转移诊断的准确性稍高于CT,分别为85.0%和70.0%;⑥PET在评价传统的三维定位适形放射治疗及局部病灶消融疗法的疗效以及随访等方面发挥重要作用.结论18F-FDG PET可用于肺癌的诊断、分期、评价疗效及随访.

Abstract:Objective To ascertain the status of positron-emission tomography(PET)for assessment of diagnosis,staging treating effects and following up in patients with lung cancer.Methods The clinical data of fifty-six patients with lung cancer were retrospectively analysed. 18 F-FDG PET scan was performed on all patients.Boundary line of standard uptaking value(SUV)for all lesions was 2.5.Results The sensitivity of 18 F-FDG PET scan in this group was 92.8%;the specitivity of PET was 89.3%;the accuracy of PET diagnosis for lung cancer was 91.0%;the false-positive rate of them accounted for 5.4%,however,false-negative rate accounted for 3.6%;the accuracies on the mediastinal lymph nodes metastases of lung cancer for PET and CT were 85.0%and 70.0%,respectively,and PET response could significantly be useful in the assessing effects on three dimension conformal radiotherapy(3DCR),local lesions ablation therapy after treatment,and following up for patients with lung cancer.Conclusion 18 F-FDC PET might accurately be useful in the diagnosis,staging,assessing effects after treatment and following up for lung cancer.
