颅脑损伤患者血糖和糖化血清蛋白联合检测的意义

目的评价颅脑损伤患者血糖和糖化血清蛋白联合检测的意义。方法检测了25例糖尿病颅脑损伤患者和25例无糖尿病颅脑损伤患者血糖（Glu）、糖化血清蛋白（GSP）水平变化,并与25例健康对照组做了比较。结果糖尿病颅脑损伤组Glu和GSP水平均高于健康对照组、无糖尿病颅脑损伤组（P〈0.01～0.05）;无糖尿病颅脑损伤组Glu水平高于健康对照组（P〈0.01）。无糖尿病颅脑损伤组治疗后血清Glu水平为（5.6±1.5）mmol/L明显低于入院时Glu水平（11.2±2.9）mmol/L,差异具有显著统计学意义（P〈0.01）。随访观察6个月,无糖尿病颅脑损伤组恢复良好率（52.00%）明显高于糖尿病颅脑损伤组（24.00%）,差异具有统计学意义（P〈0.05）。结论检测Glu水平可辅助判断颅脑损伤的严重程度,对评估预后及指导治疗具有一定意义;血清GSP检测对重型颅脑损伤患者应激性或糖尿病性血糖增高具有鉴别价值,也可作为检测重型颅脑损伤患者是否有糖尿病的一种方法。

Clinical Values of Combined Detection of Blood Glucose and Glycosylated Serum Protein in Diabetes Patients with Traumatic Brain Injury

Objective To evaluate the clinical significance of the combined detection of blood glucose and glycosylated serum protein in traumatic brain injury patients.Methods The levels of blood glucose（Glu）,glycosylated serum protein（GSP） levels were detected in 25 diabetic patients with traumatic brain injury and 25 traumatic brain injury patients without diabetes and 25 healthy cases as control group.Results The levels of Glu and GSP in diabetes patients with traumatic brain injury were higher than those of traumatic brain injury patients without diabetes,and healthy control group（P〈0.01-0.05）.The levels of Glu in traumatic brain injury patients without diabetes before and after treatment were（11.2±2.9） mmol/L and（5.6±1.5） mmol/L with significant difference（P〈0.01）.After six-months follow-up,the good recovery rate of diabetic patients with traumatic brain injury and traumatic brain injury patients without diabetes were 52.00% and 24.00% with significant difference（P〈0.05）.Conclusion The levels of Glu can judge the severity of brain injury,the serum GSP in patients with severe brain injury or stress-induced hyperglycemia in diabetic with a differential value can detect whether patients with severe brain injury have diabetes or not.
