恙虫病105例临床分析

目的探讨恙虫病的临床特点,为临床诊断与治疗提供依据。方法回顾性分析105例恙虫病住院患者的临床资料。结果本组患者男多于女,所有患者均有草地等野外活动史,均有高热。98例（93.33%）溃疡或焦痂,17例（16.19%）淋巴结肿大,45例（42.86%）肝功能损伤,21例（20%）肾功能损伤,12例（11.43%）血小板不同程度降低,29例（27.62%）外斐试验阳性。经氯霉素或四环素类药物治疗后均治愈。结论恙虫病临床表现多样,误诊率高,详细询问病史,认真体格检查并结合流行病学资料是减少误诊的关键。

Clinical Analysis on 105 Cases of Tsutsugamushi Disease

Objective To analyze the clinical feature of tsutsugamushi disease and guide the diagnosis and management of the disease. Methods The Clinical data of 105 patients with tsutsugamushi disease were retrospectively investigated. Results The male were more than female in this group of patients,all patients had the history of contacting grass and brushwood,and showed hyperpyrexia. In addition,there were 98 cases of ulcer or eschar,17 cases of lymphadenectasis,45 cases of liver injury,21 cases of renal injury,12 cases of varying degrees of thrombocytopenia,29 cases positive of Weil-Felix reaction. All cases were cured by chloramphenicol or Tetracyclines. Conclusion The clinical manifestations of tsutsugamushi disease are complicated. There is the high rate of misdiagnosis. Detailed inquire of medical history,strict physical examination combined with epidemiological information is the key to reduce misdiagnosis.
