直肠超声引导下无水乙醇注射治疗前列腺增生

目的对直肠超声引导下无水乙醇注射治疗前列腺增生进行疗效评价。方法对42例因前列腺增生导致反复尿潴留患者行直肠超声引导下无水乙醇注射治疗,并于术前,术后3个月,6个月对最大尿流率,前列腺体积,国际前列腺症状评分（IPSS）,残余尿进行测定。结果治疗后6个月,最大尿流率由术前无自主排尿提高到（13.2±3.9）m l/s,前列腺体积为（54.2±5.3）m l缩小至（40.5±4.2）m l（P〈0.01）,国际前列腺症状评分由术前的（29.8±3.0）分下降为（8.0±2.8）分（P〈0.01）,残余尿由术前的（140±50）m l降为（36.2±13.5）m l（P〈0.01）。结论经直肠超声引导下注射无水乙醇治疗前列腺增生简便易行,疗效显著且并发症少,尤其适用于不能耐受手术或拒绝手术者。

Injection of Dehydrated Ethanol in Treatment of Prostatic Hyperplasia Guided by Trans-Rectal Ultrasound

Objective To evaluate the curative effect of injection of dehydrated ethanol in treatment of prostatic hyperplasia guided by trans-rectal ultrasound.Methods Dehydrated ethanol was injected trans-rectal into 42 patients with prostatic hyperplasia with again and again urine retention.Before operatin and postoperative 3 month,6 months,evaluating the parameters which include peak urine flow,symptom score,residual urine volume,prostate volume.Results The mean peak urine flow rate increased to 13.2±3.9ml/s from no self-determination micturition;mean prostate volume decreased from 54.2±5.3ml to 40.5±4.2ml（P〈0.01）;mean symptom score decreased from 29.8±3.0 to 8.0±2.8（P〈0.01）;mean residual urine volume decreased from 140±50ml to 36.2±13.5ml（P〈0.01）.Conclusion This new approach for ethanol injection was proved to be a safe and simple procedure.Our preliminary data suggest that this technique can be performed as an outpatient procedure.It can be used as an alternative treatment for prostatic obstruction due to benign hyperplasia,especially when there is a high operate risk.
