摘 要: | 目的研究舒氧康对创伤性急性肺损伤的治疗作用。方法选择60例创伤性急性肺损伤患者,随机分为舒氧康治疗组(n=30)和常规治疗组(对照组,n=30),对照组接受常规治疗。舒氧康治疗组在常规治疗基础上静脉滴注舒氧康1500 ml,0.3 ml/(kg.min),1次/d,连用3 d,观察两组的临床疗效并检测治疗前及治疗第2、3天的血清TNF-α、IL-6和IL-8水平变化。结果舒氧康治疗组与对照组在治疗前TNF-α、IL-6和IL-8水平无差异。治疗后舒氧康治疗组的TNF-α、IL-6和IL-8水平下降较对照组明显(P〈0.05)。舒氧康治疗组治疗后呼吸频率(RR)、氧分压(PaO2)、二氧化碳分压(PaCO2),氧合指数(PaO2/FiO2)与对照组差异有统计学意义(P〈0.05)。结论舒氧康可显著抑制机体炎症介质的产生,创伤性急性肺损伤早期应用治疗效果明显。

Abstract: | Objective To observe the therapeutic value in traumatic acute lung injury using oxygenator.Methods 60 traumatic acute lung injury patients were randomly divided into oxygenator treatment group(n=30)and routine treatment group(n=30).Both groups were given routine treatment,while the patients of the oxygenator treatment group were given oxygenator(1 500ml,0.3ml.kg-1·min-1intravenously drip,once every 24 hours,and continued for 3 days)in addition.The efficacy of the therapy in both groups were assessed and serum TNF-α,IL-6 and IL-8 levels were measured on admission and on the two and third day in both groups.Results The TNF-α,IL-6 and IL-8 levels were not different both in oxygenator group and routine treatment group before treatment.The TNF-α,IL-6 and IL-8 levels reduced more significantly after treatment in oxygenator group than routine group(P〈0.05).The RR、PaO2、PaCO2 and PaO2/FiO2 after treatment in two groups were significantly different(P〈0.05).Conclusion Oxygenator significantly inhibits the production of inflammatory cytokinases.The early adminstration of oxygenator can play an important role in the treatment of acute lung injury. |
