老年胸痛患者的心理状态及其临床意义

目的观察老年冠心病和非典型胸痛患者冠状动脉造影术前心理状态及其临床意义。方法采用横断面研究,整群抽样法,对在我院进行冠状动脉选影术（CAG）检查的59例老年胸痛患者术前使用汉密尔顿焦虑量表（HAMA）、汉密尔顿抑郁量表-17项（HAMD-17）进行心理状态评估,观察老年冠心病和非典型胸痛患者CAG术前焦虑抑郁等情绪改变;同时,结合CAG结果,比较CAG阳性与阴性老年患者在一般情况和心血管危险因素方面的差异。结果59例老年患者中术前有焦虑症状者26例,占44.1%;有抑郁症状者10例,占16.9%;既有焦虑又有抑郁者8例,占14.2%。CAG检查阳性者43例,阴性者16例,CAG阴性率27.59%。CAG阳性组焦虑症状较CAG阴性组更明显（HAMA平均分：16.1±7.2比11.1±6.6,P〈0.05）。结论老年冠心病和非典型胸痛患者的焦虑、抑郁症状发生率均较高。

Psychological Status of Aged Patients with Chest Pain and Its Clinical Significance

Objective To observe the psychological status of aged patients with coronary artery disease（CAD） and atypical chest pain before coronary angiography（CAG）,and explore its clinical significance.Methods 59 cases of aged patients with chest pain and scheduled for CAG were selected by cluster sampling and cross-sectional method.The mental status was measured by Hamilton Anxiety Rating Scale（HAMA）and Hamilton Depression Rating Scale-17（HAMD-17）24 hours before CAG,and the risk factors for CAG were also determined.Results There were 26 patients whose HAMA score≥14,10 patients whose HAMD-17 score≥14 and 8 patients with both scors≥14.CAD Was diagnosed in 42 patients by CAG.HAMA score was significantly higher in patients with CAD than patients without CAD（14.1±7.2 vs 11.1±6.6,P〈0.05）.Conclusion Incidences of anxiety and depression were high in aged patients with CAD and atypical chest pain.
