新鲜厚猪皮在肉芽创面上的应用

目的了解新鲜厚猪皮片在肉芽创面上的应用效果。方法52例病人的肉芽创面随机分两组：治疗组26个肉芽创面,其中新鲜肉芽创面8个,水肿及老化弛缓性肉芽创面18个。创面经清创后采用新鲜厚猪皮片覆盖;对照组26个肉芽创面,新鲜肉芽创面7个,老化水肿肉芽创面19个。创面清创后使用油纱布覆盖。观察两组的止痛作用、不同时间段肉芽创面大体上的变化、创面的渗出情况及换药次数。结果治疗组无明显渗出,换药次数少。新鲜厚猪皮片覆盖创面后止痛作用明显。3 d后新鲜厚猪皮片下水肿老化肉芽变为鲜红色、臻密、触之易出血,新鲜肉芽无明显改变,5 d后掀起猪皮片见老化水肿肉芽及新鲜肉芽的组织量均变浅、呈鲜红色。9 d后新鲜厚猪皮片与创面黏附紧,掀起猪皮片见各类肉芽创面的肉芽组织量均减少,创基呈浅红色。对照组渗出多、换药次数频繁。清创换药后疼痛持续时间长,两组间有显著统计学差异（P〈0.01）。老化水肿肉芽无明显改变,新鲜肉芽创面呈水肿肉芽创面。结论新鲜厚猪皮片覆盖肉芽创面有止痛作用、创面渗出少、老化肉芽创面转化为新鲜肉芽创面,利于植皮。

Application of Thick Pig Skin Graft for Granulation Wound

Objective To investigate the application effect of thickness pig＇s skin graft for granulation wound.Methods 52 patients with granulation wound were randomly divided into two groups.The treatment group had 26 patients with 8 health granulation wound and 18 edematous or flaccid granulation wound.The control group had 26 patients with 7 health granulation wound and 19 edematous granulation wound.After the granulation wound were cleaned by 3% hydrogen peroxide 1‰ benzalkonium bromide and saline,the treatment group were covered by thick full pig skin graft,the control group were covered by petrolatum gauze.VAS pain score,granulation wound and wound exudation were observed.Results Compared with the control group,the granulation wound of treatment group showed more obvious exudation reduction,pain relief.The edematous granulation in treatment group（thick pig skin graft covering granulation wound） changed fresh granulation 3-5 days after treatment.The granulation reduced 9 days after treatment.The fresh granulation in the control group changed edematous granulation.Conclusion The thick pig skin graft covering granulation wound can promote the growth of health granulation,eliminate edematous granulation,clean granulation wound,get good pain relief effect.
