消化性溃疡单纯修补术后临床疗效的观察

目的观察消化性溃疡单纯修补术后加三联疗法的疗效。方法对2002年至2007年收治的消化性溃疡穿孔64例病人行单纯缝合修补术治疗，术后常规抗幽门螺旋杆菌治疗，并随访观察。结果术后3个月，经电子胃镜检查溃疡愈合率治疗组为95．1％（39／41），对照组为82．6％（19／23）；Hp阳性率治疗组为14．6％（6／41），对照组为56．5％（13／23）。术后1年，经电子胃镜检查溃疡复发率治疗组为7．3％（3／41），对照组26％（6／23）；Hp阳性率治疗组为48．8％（20／41），对照组为69．6％（16／23）。治疗组的术后溃疡复发率显著低于对照组P〈0．05。结论消化性溃疡穿孔单纯修补术安全、操作时间短、术后并发症少．术后如果能接受全程内科治疗。效果满意。

Clinical Evaluation of the Effect of Simple Closure Operation for Perforated Peptic Ulcer

Objective To evaluate the effect of simple closure operation followed by triple therapy for perforated peptic ulcer. Meth- ods 64 cases of perforated peptic ulcer treated by simple closure operation with postoperative triple therapy were followed-up and analyzed from 2002 to 2007. Results 3 month after operation, the healing rate of peptic ulcer was 95.1% in therapy group and 82.6% in the control group. 1 year after operation, the recurrence rate of peptic ulcer was 7.3% in the therapy group and 26% in the control group. The recurrence rate of therapy group was significantly higher than that of the control group （P 〈0.05）. Conclusions Simple closure operation for perforated peptic ulcer is a safe and simple method, which promises a goog prognosis for the patiens who accept eradication of H. pylori infection.
