替米沙坦联合螺内酯治疗充血性心力衰竭的临床研究

目的观察替米沙坦联合螺内酯对充血性心力衰竭患者的疗效。方法选择我院陈旧性心肌梗死合并心力衰竭患者80例随机分为2组，治疗组给予替米沙坦和螺内酯，对照组单用替米沙坦。观察2组治疗前后心脏超声、血浆脑钠肽（BNp）及心功能改变情况。结果治疗6个月后，2组心功能分级、血浆BNP和心脏超声检查各项指标与治疗前比较有明显改善（P〈0．05），治疗后2组心脏超声、血浆BNP比较有明显统计学差异（P〈0．05），但心功能分级在治疗后无明显统计学差异（P〉0．05）。结论替米沙坦联合螺内酯可以明显降低陈旧性心肌梗死合并心力衰竭患者的血浆BNP水平，改善心脏功能，替米沙坦和螺内酯对陈旧性前壁心肌梗死合并心力衰竭患者的治疗具有协同作用。

A clinical research on Telmisartan combined with Spironolactone in treatment of congestive heart failure

Objective To investigate the effect of Telmisartan combined with Spironolactone in treatment of congestive heart failure.Methods 80 patients hospitalized with heart failure due to Old anterior myocardial infaretion were randomly assigned to two groups. The treatment group was treated with Telmisartan and Spironolactone, and the solo group was treated with Telmisartan only. Before and after treatment the cardiac ultrasound ,plasma BNP and cardiac function were detected in both group.Result The index of the cardiac ultrasound ,plasma BNP and cardiac function improved significantly after 6 month of treatment ：in both group （P〈0.05）,the index of the cardiac ultrasound ,plasma BNP has statistically significant difference between the two groups after 6 month of treatment （P〈0.05）,but the index of cardiac function has no statistically significant difference between the two groups after 6 month of treatment （P〉0.05）. Conclusion The combination of Telmisartan and Spironolactone can lower the plasma BNP of Old anterior myocardial infarction with heart failure patients, and can improve the cardiac function. Telmisartan and Spironolactone has a synergistic effect on Old anterior myocardial infarction with heart failure patients.
