对比增强磁共振血管成像诊断先天性主动脉弓部畸形

摘    要:目的 评价对比增强磁共振血管成像诊断先天性主动脉弓部畸形 (包括主动脉缩窄、主动脉弓中断、主动脉弓褶曲等 )的临床价值。方法 回顾 17例手术证实存在主动脉弓部畸形的患者 ,先后行对比增强磁共振血管成像检查和超声检查 ,评价MR检查图像质量 ,比较二种检查方法诊断准确性 ,两种方法测量的数据与术中病理结果对比进行统计学分析。结果 对比增强磁共振血管成像检查图像质量较高 ,诊断各种主动脉弓部畸形准确率略优于超声 ,测量11例主动脉缩窄和弓中断患者的畸形位置和主动脉内径与术中病理相比均无显著性差异 (P >0 .0 5 )。结论 尽管样本量较小 ,但可以初步证实对比增强磁共振血管成像技术在诊断主动脉弓部畸形一类疾病中有良好的应用前景。

Abstract:Objective To evaluate the clinical value of contrast-enhanced MR angiography (CE MRA) for congenital aortic arch abnormality, including coarctation of aorta, interrupted aortic arch, etc.Methods We Reviewed 17 cases of abnormality in aortic arch who underwent operation therapy and examined them by CE MRA and echocardiography pre-operation. We alsoassessed the image quality of CE MRA, compared the accuracy of two methods as well as analysed the measurement results and operation pathology statistically.Results The image quality of CE MRA was well in diagnosis and the accuracy was a little better than echocardiography. The sites of abnormality and diameter of ascending aorta in 11 cases with aortic coarctation or interrupted aortic arch had no significant difference when compared between CE MRA and pathological results.Conclusion Although the sample is small in this study, we think the CE MRA will have a good future in diagnosis congenital aortic arch abnormality.
