重型颅脑损伤手术患者死亡原因分析

目的：回顾性总结重型颅脑损伤患者术后死亡原因和救治经验。方法：分析GCS3-8分的119例重型颅脑损伤手术患者的术前伤情、血肿类型、处理措施和预后。结果：119例中死亡35例。主要死亡原因：严重脑挫裂伤、颅内血肿、脑疝、脑干损伤，严重的复合伤、合并症、并发症。结论：早期诊断、早期处理，正确掌握手术指征，有效解除颅内高压，积极治疗复合性损伤及防治并发症是提高治愈率、降低病死率的关键。

Analysis of cause of postoperative death in patients with severe craniocerebral injury

Objective： to analyze retrospectively the cause of postoperative death in the patients with sever craniocerebral injury and summarize management experience. Methods： The clinical data of 119 patients of GCS 3-8 with severe craniocerebral injury were analyzed in regard to the degree of trauma, the types of hematomas, the management and progosis, and all the patients have received operation. Results： 35 patients died in all 119 patients. The deaths mainly caused by severe contusion and laceration of the brain,intracranial hematomas, herniation of brain,brain stem injury, severe associated injury and complication. Conclusion： Early diagnosis, timely treatment, effective decompressions for intracranial pressure and correct management of complication are essential measures to promote cure rate and lower mortality rate.
