胰管空肠吻合在胰十二指肠切除术中的应用研究

摘    要:【目的】评价连续胰管空肠吻合方法在胰十二指肠切除术中的应用价值。【方法】回顾分析2004年12月至2011年2月间在我院行胰十二指肠切除术的患者58例,采用连续胰管空肠吻合方式的患者32例作为胰管吻合组,采用传统胰肠端端套入式吻合方式的患者26例作为传统吻合组。比较两组的手术时间、术中出血、术后胰瘘和术后住院时间等情况。【结果】术后胰瘘发生情况胰管吻合组与传统吻合组比较差异未见统计学意义(P〉0.05),手术时间、术中出血、术后住院时间等方面与传统吻合组比较差异有统计学意义(P〈0.05)。【结论】在行胰十二指肠切除手术时胰肠吻合应用连续胰管空肠吻合方式能明显缩短手术时间,降低胰瘘的发生率,患者术后恢复快,值得临床推广。

Abstract:【Objective】To evaluate the application value of pancreaticojejunostomy in pancreaticoduodenectomy.【Methods】A total of 58 cases undergone pancreaticoduodenectomy from Dec.2004 to Feb.2011 were reviewed. All of these patients were divided into two groups randomly. The treatment group(n = 32)were received continuous pancreaticojejunostomy,while the control group(n = 26) received the conventional pancreaticoenterostomy. Operation time,blood loss,occurrence of postoperative pancreatic fistula and the time of hospitalization were observed.【Results】Compared with control group, there was no significant difference in pancreatic fistula (P 0.05),but there were significant difference in operation time,blood loss and the time of hospitalization(P 0.05).【Coclusion】 Pancreaticojejunostomy in pancreaticoduodenectomy could reduce the operation time,solve the problem of pancreatic fistula and make recovery of patient faster, it is worth to be used widely in clinical.
