妊娠合并血小板减少95例临床分析

目的探讨妊娠合并血小板减少（PT）的病因、围生期处理及妊娠结局。方法对2006年1月至2009年10月收治的95例妊娠合并血小板减少患者的临床资料作回顾性研究。结果妊娠合并血小板减少（PT）的病因主要是：妊娠特发性血小板减少（PAT）48例（占50.53%）、妊娠期高血压疾病20例（21.05%）、特发性血小板减少性紫癜10例（10.53%）等。对血小板≤50×10^9L^-1的患者采用糖皮质激素和（或）丙种球蛋白治疗,分娩前后使用血小板制剂,同时考虑剖宫产。结论妊娠特发性血小板减少是最常见的妊娠合并血小板减少症类型。在治疗原发病的基础上,糖皮质激素、丙种球蛋白、血小板输注是治疗严重血小板减少的有效手段。对分娩前血小板≤50×10^9L^-1的患者重点监护,可适当放宽剖宫产指征确保母婴安全。

Clinical analysis of 95 pregnant women complicated with thrombocytopenia

Objective To explore the etiopathogenesis and optimal management in pregnant women complicated with thrombocytopenia.Methods the clinical data of 95 cases of pregnant women complicated with thrombocytopenia in our hospital from 2006 to 2009 were reviewed.Results for this reason an optimal management approach was used when there was a pregnancy associated with thrombocytopenia(PAT) found 48 cases(50.53%),hypertensive disoraers in pregnancy in 20 cases(21.05%),idiopathic thrombocytopenia(ITP) found 10 cases(10.53) and so on.Glucocorticoid,immunoglobulin and platelet transfusion should be given to those with platelet count less than 50×109L-1,especially before delivery,a cesarean section should be also considered in the meanwhile.Conclusion Pregnancy-associated thrombocytopenia(PAT) was most prevalent cause in pregnant women complicated with thrombocytopenia.Glucocorticoid,immunoglobulin and platelet transfusion are effective methods for PT in addition to the treatment for the primary disease.To the patients with platelet count less than 50×109L-1 before delivery with suitable monitor.Cesarean section should be performed to ensure the safety throughout the prenatal period.
