COPD并急性呼吸衰竭患者微循环及血流变指标的变化规律研究

目的:探讨COPD并急性呼吸衰竭患者微循环及血流变指标的变化规律。方法:选取2010年9月～2012年6月于本院进行治疗的26例COPD并急性呼吸衰竭患者为A组,同期的26例COPD不伴有呼吸衰竭的患者为B组,26名健康体检人员为C组,后将三组人员中甲襞微循环、微循环指标及血液流变学指标进行比较。结果:A组的管襻形态、血液流态、襻周状态及总积分均高于B组和C组,管襻畸形率及红细胞聚集、白微栓均高于B组及C组,毛细血管流速、血液流速、襻顶血管直径低于B组及C组,血液流变学指标均高于B组及C组,而B组则均差于C组,同时重度呼衰患者则差于轻度患者P均<0.05,均有显著性差异。结论:COPD并急性呼吸衰竭患者微循环处于较差的状态,而血流变指标则处于较高的状态。

Variation of microcirculation and hemorheology indexes of patients with COPD complicated with acute respiratory failure

Objective: To study the variation of microcirculation and hemorheology indexes of patients with COPD and acute respiratory failure.Methods:Twenty six patients with COPD and acute respiratory failure that were treated in our hospital from September 2010 to June 2012 were selected as group A,26 patients with COPD were choose as group B,26 healthy personnel were selected as group C,then the nail-fold microcirculation,microcirculation and hemorheology indexes of three groups were compared.Results:The loop morphology,blood flow pattern,loop cycle state and total integration of group A were all lower than those of group B and group C;while loop deformity rate,blood speed,erythrocyte aggregation and white micro thrombus were all higher than those of group B and group C,capillary flow and loop top vessel diameter were all lower than those of group B and group C.Hemorheology indexes were all lower than those of group B and group C.Of the latter two groups,hemorheology indexes of group B was worse than that of group C.Hemorheology indexes of patients with severe respiratory failure were worse than those with mild respiratory failure(P all <0.05).Conclusion:The microcirculation indexes of patients with COPD and acute respiratory failure is in poor condition,and the hemorheology indexes are in higher level.
