脑血管病合并睡眠障碍患者的认知行为和运动功能的临床研究

摘    要:目的:探索脑血管病合并睡眠障碍患者的认知、行为和运动功能的临床特点.方法:2015年6月至2016年6月因脑血管病来我院住院治疗的患者226例,根据患者是否伴有睡眠障碍分为脑血管病合并睡眠障碍组(研究组)和脑血管病未合并睡眠障碍组(对照组),分析两组患者的认知、行为和运动功能差异.结果:研究组患者的匹兹堡睡眠质量量表(PSQI)和爱泼沃斯嗜睡量表(ESS)得分高于对照组(P<0.01).研究组患者的简明精神状态检查量表(MMSE)、Fugl-Meyer运动量表(FMA)和认知功能评定量表(LOTCA)得分低于对照组(P<0.05).研究组患者在躯体化、强迫症状、人际关系敏感、抑郁、焦虑、恐怖方面得分高于对照组(P<0.05).研究组生活质量量表(GQOL-74)得分在认知功能、心理功能、躯体功能和社会功能上均低于对照组(P<0.01).结论:脑血管病合并有睡眠障碍的患者存在较严重的日间嗜睡和夜间失眠,同时此类患者认知、运动功能丧失较多,心理健康状况较差,生活质量较同类型疾病患者下降明显,临床治疗和护理上应予以重视.

Abstract:Objective:To explore the clinical features of cognitive, behavioral and motor function in pa-tients with cerebrovascular disease complicated with sleep disorders. Methods: A retrospective study on 226 cases patients with cerebrovascular disease admitted in our hospital from June 2015 to June 2016, according to whether patients with sleep disorders as cerebrovascular disease and sleep disorder group ( study group) and cerebrovascular disease group (control group).The differences in behavior and motor function of the two groups of patients, cognitive were analyzed. Results:Sleep quality scale ( PSQI) and sleepiness scale ( ESS) scores were significantly higher in the study group than in the control group (P<0.01) in the study group (). The simple mental state examination scale ( MMSE) , Fugl-Meyer Motor Scale ( FMA) and cognitive function rat-ing scale ( LOTCA) score in the study group were significantly lower than those in the control group ( P<0. 05) . Patients in the study group were significantly higher than those in the control group ( P<0.05) in the as-pects of somatic, obsessive compulsive symptoms, interpersonal sensitivity, depression, anxiety and terror. The scores of quality of life scale ( GQOL-74) in the study group were significantly lower than those in the control group ( P<0.01) in cognitive function, mental function, physical function and social function. Conclu-sion:Cerebrovascular disease complicated with sleep disorders in patients with severe daytime sleepiness and nocturnal insomnia, and cognitive and motor function in patients with such loss more, poor mental health, quality of life of patients decreased significantly compared with the same type of disease, clinical treatment and nursing care should be paid attention to.
