原发性输卵管癌11例临床分析

目的:分析原发性输卵管癌的临床特点,提高临床医生对原发性输卵管癌的认识。方法:回顾性分析11例原发性输卵管癌的发病年龄、临床表现、辅助检查及治疗方法。结果:11例患者平均年龄57.9岁,8例(72.7%)为绝经后女性。最常见的临床表现依次为盆腔包块9例(81.8%),腹痛5例(45.5%),阴道流血4例(36.4%),腹胀2例(18.2%),异常阴道流血伴排液2例(18.2%),阴道排液1例(9.1%)。术前接受血清癌抗原125(CA125)测定的9例中,5例(55.6%)高于35U/ml。11例中术前仅1例诊断为输卵管癌,误诊率达90.9%。结论:原发性输卵管癌的发病率低,临床表现多样且缺乏特异性,术前明确诊断困难,辅助检查有助于诊断,手术是主要的治疗手段。

Clinical analysis on 11 cases with primary carcinoma of Fallopian tube

Objective:To analyze the clinical characteristics of primary carcinoma of Fallopian tube,and improve the cognition of clinicians to the disease.Methods:The onset age,clinical manifestations,accessory examination,and therapy of 11 cases with primary carcinoma of Fallopian tube were analyzed retrospectively.Results:The mean age of 11 cases was 57.9 years old,and eight cases(72.7%) were postmenopausal women.The most common clinical manifestations from high to low were pelvic mass(9 cases,81.8%),abdominal pain(5 cases,45.5%),vaginal bleeding(4 cases,36.4%),abdominal distension(2 cases,18.2%),abnormal vaginal bleeding combined with vaginal drainage(2 cases,18.2%),vaginal drainage(1 case,9.1%).Among nine cases receiving serum cancer antigen 125(CA125) detection before surgery,the levels of CA125 in five cases(55.6%) were more than 35 U/ml.Among 11 cases,only one case was diagnosed as primary carcinoma of Fallopian tube,the rate of misdiagnosis was 90.9%.Conclusion:The incidence tate of primary carcinoma of Fallopian tube is low,the clinical manifestations are various and lack specificity,the preoperative diagnosis is difficult,accessory examination is helpful to diagnosis,surgery is the main therapeutic method.
