细胞DNA定量分析技术在宫颈癌筛查中的回顾性研究

目的:比较和评估两种宫颈癌筛查方法的应用价值。方法:选取进行宫颈癌检查并有活检结果的451个病例,均为宫颈刷取材、液基方法同时制两张片,一张进行巴氏染色用于常规细胞学TBS诊断,另一张经Feulgen染色用于细胞DNA定量分析检测。临床活检取材均在阴道镜指导下进行。结果:以活检结果作为金标准,451例中共有宫颈癌1例,CINⅢ/原位癌3例,3例CINⅡ及15例CINⅠ,其余均为慢性宫颈炎。以活检结果≥CINⅡ作为宫颈癌前病变的阳性标准,采用细胞DNA定量分析检测的敏感性和特异性分别为100.00%与95.50%,而常规细胞学检测的敏感性和特异性分别为42.86%与99.01%。同一个标本同时采用两种方法联合检测,敏感性可提高到100.00%。结论:细胞DNA定量分析技术可与常规细胞学联合开展应用在宫颈癌筛查中。

Restrospective study on application of DNA image cytometer in the cervical cancer screening

Objective:To compare and evaluate the application value of two methods for cervical cancer screening. Methods:451 women were reviewed from the people who screened from March 2011 to March 2013.The cytology samples were taken by cervix brush and prepared for 2 slides: one was read by cytologist and the other was tested by DNA image cytometer.The biopsy was taken with direction of colposcopy. Results:451 biopsy results had been proved,including 1 cancer cases,3 CINⅢ/CIS,3 CINⅡ and 15 CINⅠ.The sensitivity and specificity of DNA image cytometry were 100.00% and 95.50%,the cytology of both of them was 42.86% and 99.01% respectively.The sensitivity could be increased to 100% by combination of two methods. Conclusion:DNA image cytometry can be used in cervical cancer screening combined with cytology.
