子宫动脉栓塞术用于中央性前置胎盘引产10例分析

目的:探讨子宫动脉栓塞(UAE)联合依沙吖啶(利凡诺)羊膜腔注射用于中央性前置胎盘中晚期妊娠胎儿畸形引产的可行性及疗效。方法:10例中央性前置胎盘(或胎盘前置状态)患者先行依沙吖啶(利凡诺)羊膜腔注射,再行超选择性子宫动脉栓塞术,分析其分娩方式、阴道出血、并发症、月经恢复情况。结果:10例患者中,8例经阴道分娩,2例行剖宫术取胎;8例经阴道分娩患者阴道出血量平均为190 ml,2例剖宫取胎患者产后出血量均为200 ml;2例患者术后出现发热,经抗炎治疗及终止妊娠后体温降至正常,未出现其他并发症;引产后平均月经恢复时间为37天。结论:子宫动脉栓塞联合依沙吖啶羊膜腔注射用于中央性前置胎盘胎儿畸形引产是一种安全、创伤小、疗效好的可靠方法,值得推广。

Analysis on uterine arterial embolization (UAE) used for induced labor of central placenta previa in 10 cases

Objective:To explore the feasibility and therapeutic effect of uterine arterial embolization(UAE) combined with amniotic cavity injection for central placenta previa and fetal abnormality during the second and the third trimesters of pregnancy.Methods:Ten patients with central placenta previa(placenta previa state) were treated with intra-amniotic membrane injection of rivanol and simultaneously selective uterine artery embolization.The delivery modes,vaginal bleeding,complications,and menstrual recovery time were analyzed.Results:Among the ten patients,eight cases received vaginal delivery,two cases received cesarean section;the amount of vaginal bleeding was 190 ml on average for the cases receiving vaginal delivery,the amount of postpartum hemorrhage was 200 ml for the cases receiving cesarean section;two patients had fever after operation,the temperature dropped to normal after anti-inflammatory treatment and termination of pregnancy,no other complications occurred;the average recovery time of menstruation was 37 days.Conclusion:UAE combined with amniotic cavity injection is a safe,small trauma,good curative effect,and reliable method for cases with central placenta previa and fetal deformity during the second and the third trimesters of pregnancy,which is worthy to be popularized.
