75例糖尿病合并蛋白尿患者的病因分析

目的：分析糖尿病患者合并蛋白尿的原因、发生率及其与病程的关系，给临床提供有效的防治措施。方法：对７５例糖尿病合并蛋白尿患者的清晨第１次中段尿测定尿蛋白的含量，并统计各种病因对蛋白尿发生的影响。结果：７５例糖尿病患者合并蛋白尿的病因中以合并泌尿系感染者占首位，依次为糖尿病肾病、高血压、心功能不全、酮症、高渗昏迷和休克。其中因一种原因出现蛋白尿者占９．３％，２种原因并存者占２４．０％，３种及３种以上原因并存者占６６．７％；且病程越长，尿蛋白含量越高。结论：糖尿病患者合并蛋白尿常为多因素所致，尿蛋白含量与预后关系密切。

Etiological analysis on 75 cases with diabetes mellitus complicating proteinuria

Objective :To analyse the etiology,occurance rate,and relationship with ill course in patients with diabetes mellitus complicating proteinuria so that to designate effectively preventive and therapeutic methods. Methods :In the early morning the first middle segmental urine was measured in 75 cases with diabetes mellitus complicating proteinuria and using computerized high2speed analytical instrument of urine the volume of urinary protein was measured,the influences of different causes on occurance of proteinuria was count. Results :In 75 cases with diabetes mellitus the causes complicating proteinuria were following the first was complicated by infections of urinary system,then in turn diabetic nephropathy,hypertension,cardiac insufficiency,diabetic ketosis,hyperosmolar coma and shock.Among these reasons 93% was only due to one reason,240% due to concomitant two reasons,667% due to three or more than three reasons and the longer the ill course was the higher the volume of urinary protein was. Conclusions :The courses for patients with diabetes mellitus to complicate proteinuria are multiple factors and the relationship between volume urinary protein and prognosis is very closer.
