尿微量白蛋白、血清胱抑素、尿视黄醇结合蛋白在糖尿病肾病不同分期检测中的临床意义

摘    要:目的通过检测糖尿病肾病不同临床分期的尿微量白蛋白(UMA)、血清胱抑素(ScysC)、尿视黄醇结合蛋白(RBP),探讨上述指标在糖尿病肾病病程进展中的意义。方法根据尿微量白蛋白排出量将117例2型糖尿病患者分为I组(正常白蛋白尿组)、Ⅱ组(早期糖尿病肾病组)、HI组(显性糖尿病肾病组),以健康体检者作为对照。检测各组的uMA、ScysC、尿RBP。结果①I组与对照组比较,UMA差异无统计学意义(P〉0.05);ScysC、尿RBP差异均有显著统计学意义(P〈0.01);②H组与对照组、I组比较,uMA、ScysC、尿RBP差异均有显著统计学意义(P〈0.01);⑧Ⅲ组与对照组、I组、Ⅱ组比较,UMA、ScysC、尿RBP差异均有显著统计学意义(P〈0.01)。结论UMA、ScysC、尿RBP均可作为糖尿病肾病病程进展的观察指标,并且在病程早期ScysC、尿RBP的敏感性高于UMA。

Clinical significance of measuring urine microalbumin,serum cystatin C and retinal binding protein in the different staging of diabetic nephropathy

Abstract:Objective To explore the significance of urine microalbumin(UMA),serum cystatin C(ScysC) and retinal binding protein(RBP) in the progression of diabetic nephropathy through detecting the results of such indicators in different clinical stages.Methods A total of 117 patients with type 2 diabetes mellitus were divided into three groups including group Ⅰ(normal albuminuria),II(early diabetic nephropathy) and Ⅲ(dominant diabetic nephropathy) according to the excretion rate of UMA,ScysC and RBP,and healthy individuals were enrolled as the control group.Results(1)There was no significant difference between group Ⅰ and the control group in UMA,whereas ScysC and RBP were of greatly significant difference(P<0.01);(2)There was significant meaning in all the indicators of UMA,ScysC and RBP in group Ⅱ compared with the control group and group Ⅰ;(3)There was significant meaning in all the indicators in group Ⅲcompared with the other groups.Conclusion UMA,ScysC and RBP are valuable indicators for the observation of the progression of diabetic nephropathy,among which ScysC and RBP are more sensitive than UMA in the early stage.
