乳腺叶状肿瘤临床病理特点及误诊分析

目的探讨乳腺分叶状肿瘤的临床病理特点、诊断、鉴别诊断及误诊原因。方法收集病理明确诊断的乳腺分叶状肿瘤9例,对其临床病理资料进行回顾性研究及误诊原因分析。结果确诊为良性、交界性和恶性叶状肿瘤各3例。术前主要误诊为纤维腺瘤、乳腺增生、青春期纤维腺瘤、乳腺癌等。结论乳腺叶状肿瘤是一组从良性、交界性到恶性的谱系性疾病,组织学形态变化较大,生物学行为不定,容易复发和恶变。临床表现和影像学检查无特征性,术前临床误诊率高,诊断主要依据病理学形态改变,免疫组化具有一定的辅助作用和鉴别诊断作用。

Clinicopathologic features and misdiagnostic analysis of phyllodes tumor of the breast

Objective To investigate the clinicopathological features,diagnosis, differential diagnosis and cause of misdiagnostic of phyllodes tumor of the breast.Methods The clinicopathological data of the 9 cases of breast phyllodes tumor were collected and analyzed the cause for misdiagnosis. Results 3 cases of benigh, 3 cases of borderline and 3 cases of malignant phyllodes tumor were misdiagnosed respectively.These cases were mainly misdiagnosed as mammary fibroadenoma,mastoplasia,juvenile mammary fibroadenoma and breast carcinoma before operation.Conclusion Phyllodes tumor of the breast was a spectrum disease from benign, borderline to malignant.They varied greatly in histomorphology and biological behaviour,and relapsed and became malignant easily.They had no specific clinical and imaging manifestations.Phyllodes tumor of the breast has the higher clinical misdiagnosis rate and frozen pathological misdiagnosis rate.They were diagnosed mainly based on pathological morphology changes.Immunohistochemical method helped in diagnosis and differential diagnosis.
