妇产科手术患者抗生素应用情况分析

目的观察妇产科手术患者抗生素应用情况,分析用药的合理性。方法抽取妇产科手术出院病历270份,分析抗生素使用情况。结果 270例患者应用抗生素(88.89%);单一抗生素25.00%、二联用药58.33%、三联用药16.67%。以β-内酰胺为主180例次,硝基咪唑类(甲硝唑、替硝唑等)110例次,大环内酯类及其他70例次,共应用抗生素的16种;预防性用药62.50%;抗生素给药频率为1次/d,共62.50%;2次/d,共25.00%;3次/d,共12.50%,预防性用药150例中围手术期40.00%,术后开始60.00%。结论妇产科手术使用抗生素的种类选择上基本符合要求,也但存在不合理应用现象,应健全抗生素应用的制度化、规范化管理,完善对抗生素应用的宏观调控机制。

Survey on situation of antimicrobial application in patients with gynecological surgery

Objective To observe the situation of antimicrobial application in gynecological surgery in this hospital,and to analyze the rationality in application of drugs.Methods Two hundred and seventy medical records of inpatients with gynecological surgery in this hospital were selected for this study in order to analyze the situation in application of antimicrobial agents.Results Among these 270 patients,240 of them（88.89%） were applied with antibiotics,including 25.00% of them with application of single antimicrobial,58.33% with 2 kinds of antibiotics and 16.67% with 3 kinds of antibiotics.In this series of cases,β-lactamases were applied in 180 instances,nitroimidazoles（metronidazole,tinidazole etc） were applied in 110 instances,macrolides in 70 instances etc.The kinds of applied antibiotics included 16 varieties.62.50% patients were applied for prophylaxis.The frequency of application of antimicrobials was 1 time/d in 62.50% of them;2 times/d in 25.00% of them,and 3 times/d in 12.50%.The application of antibiotics in 150 cases was for prophylaxis including perioperative application in 40.00% and postoperative application in 60.00%.Conclusion The application of antimicrobial dugs in gynecological surgery is basically meeting the requirement,but there are many unreasonable application of antimicrobial;and it should be improved in systematic and standardized management for antimicrobial application in order to perfect the mechanism for rational application of antibiotics.
