关节功能部位严重深度烧伤创面的皮瓣修复

目的探讨关节功能部位严重深度烧伤创面用皮瓣移植修复的临床疗效。方法对15例颈部、肢体关节部位严重深度烧伤患者,清创后即时应用局部皮瓣、带蒂皮瓣、吻合血管游离皮瓣移植修复创面。结果15例深度烧伤患者应用皮瓣移植,最大面积为18cm×9cm,全部成活,未出现感染坏死,创面一次性封闭,关节功能活动正常,效果满意。结论对于严重关节、功能部位深度烧伤,早期清创后应用皮瓣修复创面,对维持关节活动功能方面是一种安全、有效方法,并可缩短创面愈合时间。

THE REPAIR OF SERIOUS DEEP BURNT WOUNDS AT JOINT'S FUNCTION POSITION USING SKIN FLAPS

Objective To investigate the clinical therapeutic effect of the repair of serious deep burnt wounds at joint's function position using skin flaps. Methods Fifteen deep burned patients with wounds in the neck and the joints of limb function position were enrolled in this study. After local debridement, the wounds defects were repaired with local skin flaps, axial pattern skin flaps and free skin flaps immediately. Results The flaps were applied in 15 deep burned cases with the maximal defect of 18 cm×9 cm and all survived very well free from infection or necrosis. The wounds were closed primarily. The function of joints kept normal with satisfactory result. Conclusion Early debridement followed by skin flaps transplantation could be simple, safe and effective method in serious deep burnt of joints function position, and will shorten the wound healing time, keep the function of the joint.
