鼻内镜下多种入路上颌窦病变的处理

目的探讨鼻内镜下多种入路上颌窦病变的处理方法及疗效。方法采用鼻内镜中鼻道上颌窦开窗术、下鼻道上颌窦开窗术、犬齿窝入路上颌窦开窗术、上颌窦内侧壁切除术、Denker术式联合鼻内镜上颌窦病变切除术等多种入路对59例上颌窦病变进行手术治疗。结果除2例上颌窦后鼻孔息肉和1例内翻性乳头状瘤复发，经再次手术治疗痊愈外，其他患者均无复发。结论根据上颌窦病变的不同临床特点，采用不同鼻内镜手术入路既可有效治愈上颌窦病变，同时可最大限度地减少手术创伤。

Endoscopic treatment of maxillary sinus lesion with various approaches

Objective To explore the methods and results of endoscopic treatment of maxillary sinus lesion with various approaches. Methods Fifty-nine maxillary sinus lesions were treated by various endoscopic approaches including endoscopic middle meatus antrostomy, inferior meatus antrostomy, canine tooth fossa approach, endoscopic medial maxillectomy, Denker approach combined with endoscopy. Results No relapse was found except 2 antrochoanal polyp and 1 inverted papilloma. These 3 cases recovered by second operation. Conclusion Selecting various endoscopic approaches to treat maxillary sinus lesion according to the different clinical characteristics is the excellent method to reduce the surgical trauma.
