阑尾黏液性肿瘤的超声诊断

目的探讨阑尾黏液性肿瘤的B超声像图特点，提高对本病的超声诊断。方法回顾性分析上海中山医院近10年间11例及我院近期1例共12例阑尾黏液性肿瘤B超声像图特点。结果11例为右下腹囊实性肿块，囊性为主，囊壁多较厚，层次不清，可伴有钙化，囊内液性部分透声较差，可见密集点状回声及网状分隔，后壁回声增强。1例表现为实性占位者肿块瘤体较小。其中1例超声示囊性肿块囊腔内见到残存管状的阑尾结构，对提示本病有相当的帮助。3例合并腹腔积液。结论阑尾黏液性肿瘤少见且临床诊断困难，但只要注意想到该病，结合位置与声像图特点，综合各种影像手段，术前作出正确的诊断还是可能的。

Ultrasonic Diagnosis of Appendiceal Mucinous Tumors

Objective To investigate the B-uhrasound image features of appendiceal mucinous tumors and to improve the ultrasonic diagnosis of this disease. Methods Twelve cases, diagnosed pathologically as appendiceal mucinous tumors in the past ten years in both hospitals, were under retrospective analysis of the B-ultrasound image features. Results Eleven cases had sac solid masses in the right lower quadrant abdoman, and the masses were cystic-based, thick wall, unclear depth, and may be associated with calcification. Some part of the cystic fluid had poor sound transmission, dense blobshaped echo and net-like separation, and the echo of the deep wall can be enhanced. The other case showed smaller solid mass. One case was found remained appendix tubular structure in cystic cavity in the image, which prompted the diagnosis. Three cases were combined with ascitcs. Conclusion Appendiceal mucinous tumor is rare and the clinical diagnosis is difficult. A correct preoperative diagnosis is possible if we know and pay more attention to this disease, and utilize every kinds of imaging tools combined with location and sonographic features.
