内窥镜经蝶窦入路垂体瘤切除术的临床观察

目的 探讨垂体腺瘤的诊断和经鼻蝶窦显微手术的方法及经验。方法 经鼻蝶窦显微外科切除垂体腺瘤105例，其中微腺癌50例、大腺瘤27例、巨大腺瘤13例、侵袭性腺瘤15例，观察其治疗结果。结果 内窥镜下全切除肿瘤78例，近全切除13例，视力、视野均较术前改善；术后63例内分泌化验恢复正常，17例激素水平下降，3例术后出现一过性脑脊液鼻漏，2例术后出现尿崩。结论 经单鼻孔-蝶窦内窥镜下切除垂体腺瘤，创伤小。

Endoscopic resection of pituitary adenoma via sphenoid sinus:105 cases report

Objective To investigate the application of neuro endoscope in the pituitary surgery.Methods 105 patients with pituitary adenomas were surgical treated through a nostril using neuro endoscopes(50 patients with micro-adenomas,27 patients with big adenomas,13 patients with large adenomas and 15 patients with invade adenomas);Observed the results.Results 78 underwent total resection and 13 subtotal resection , rapid improvement of vision was noted postoperatively.Among the 63 patients with hormone levels with follow-down, 3 patients developed postoperative CSF leakage and 2 patients developed transient diabetes.Conclusion Endoscopic neurosurgery to treat pituitary adenomas can reduce the trauma of operation, decrease complications, and offer reliable effect.
