超敏C-反应蛋白与2型糖尿病并原发性非酒精性脂肪肝的相关性研究

摘    要:目的分析2型糖尿病(他DM)并原发性非酒精性脂肪肝(primary nonalcoholic fatty liver,PNAFL)与血清超敏C一反应蛋白(highly sensitiveC-reactive protein,hs—CRP)水平的相关性。方法我院患者83例分为:A组T2DM并PNAFL22例;B组单纯T2DM23例;C组单纯PNAFL20例;D组对照组18例,均测空腹血糖(FPG)、HbAlC、肝功、hs—Clip、BMI、WHR、Homa—IR。结果①A、B、C组hs—CRP、WHR、GGT、HbAlC、Homa-IR均高于D组(P〈0.05),A、B组高于C组(P〈0.05),A、B组比较差异无显著性(P〉0.05);②A、B、C组hs—CRP较D组明显升高(P〈0.05),A、B组hs—CRP高于c组(P〈0.05)A、B组hs—CRP比较差异无显著性(P〉0.05);③各组hs—CRP与wHR、FPG、FINS、Homa—IR均呈正相关(P〈0.05)。与hs—CRP有关的因素为:WHR、HNS、Homa—IR(P〈0.001),对hs—CRP影响的大小依次为WHR〉FINS〉Homa—IR。结论T2DM并PNAFL患者血清hs—CRP水平明显升高,hs—CRP可能在T2DM并PNAFL的发生、发展中扮演重要角色。

Investigation of the relationship between the plasma highly sensitive C-reactive protein level and patients with type 2 diabetes mellitus complicating primary nonalcoholic fatty liver

Abstract:Objective To investigate the relationship between the plasm bs-CRP level and patients with type 2 diabetes mellitus complicating primary nonalcoholic fatty liver. Methods 83 subjects divided into four groups: A group was T2DM complicating PNAFL ,22 examples;B group was pure T2DM,23 examples;C group was pure PNAFL,20 examples;D group was normal control, 18 examples.Results (1)WHR,BMI,GGT,HbA1C ,Homa-IR were higher in A,B,C groups than that in D group(P〈 0.05), it was higher in A ,B groups than that in C group (P〈0.05) ; there was no difference in comparing Agroup with Bgroup (P〉 0.05 ) ; (2)Hs-CRP was significant higher in A, B, C groups than that in D group (P〈 0.05), it was significant higher in A, B groups than that inC group (P〈0.05),it had no statistically significant difference in A ,B groups (P〉0.05). (3)tts-CRP was positive correlated to WHR, FPG, FINS, Homa-IR (P〈 0.001), and multiple linear stepwise regression analysis revealed that the selected factors, which were related to hs-CRP were WHR, FINS,Homa-IR (P〈 0.001 ).The effects of every selected factor on hs-CRP were WHR, FINS, Homa-IR by turns according to standard partial regression coefficient. Conclusions The plasma hs-CRP level is higher evidently in patients with T2DM complicating PNAFL. We presume that the hs-CRP play an important role in the happening and development of T2DM complicating PNAFL
