小剂量阿奇霉素对支气管哮喘患者呼出气NO浓度及肺功能的影响

摘    要:目的:探讨小剂量阿奇霉素对支气管哮喘患者呼出气NO浓度及肺功能的影响,评价阿奇霉素在支气管哮喘治疗中的作用。方法:选取2012年本院呼吸科首次被确诊为支气管哮喘的58例患者,按照随机数字表法将其分为阿奇霉素组和对照组各29例,阿奇霉素组采用小剂量阿奇霉素联合舒利迭进行治疗,对照组采用舒利迭治疗,两组均必要时应用吸入万托林。观察比较两组FEV1、PEF及呼出气FeNO水平的差异。结果:两组治疗前的FEV1、PEF及呼出气FeNO水平比较差异均无统计学意义(P〉0.05)。两组治疗后的FEV1及PEF水平均明显高于治疗前,且阿奇霉素组治疗后的FEV1及PEF水平均明显高于对照组,差异均有统计学意义(P〈0.05)。两组治疗后呼出气FeNO水平均明显低于治疗前,且阿奇霉素组治疗后呼出气FeNO水平明显低于对照组,差异均有统计学意义(P〈0.05)。结论:小剂量阿奇霉素联合舒利迭治疗支气管哮喘能够取得良好的治疗效果,值得临床推广。

Clinical Impacts of Low-dose Azithromycin on Lung Function and Fraction of Exhaled Nitric Oxide ;Concentration in Bronchial Asthma

Abstract:Objective:To investigate the effects of low-dose azithromycin on lung function and exhaled nitric oxide concentration in bronchial asthma.Method:58 patients with newly diagnosed asthma were selected in respiration department in 2012,they were randomly divided into the azithromycin group and the control group,29 cases in each group. The azithromycin group was treated with azithromycin combined with inhaled Salmeterol xinafoate and fluticasone propionate powder for inhalation,the control group was only treated with salmeterol xinafoate and fluticasone propionate powder, ventolin inhalation were applied when necessary,the FEV1,PEF and the level of FeNO between the two groups were observed and compared.Result:The FEV1,PEF and the level of FeNO of two groups before treatment,the differences had no statistical significance(P〉0.05).FEV1 and PEF levels after treatment in the two groups were significantly higher than those before treatment,and the FEV1 and PEF levels in azithromycin group after treatment were significantly higher than those in control group,the differences were statistically significant(P〈0.05).The level of FeNO after treatment in the two groups were significantly lower than the control group,and the level of FeNO in azithromycin group after treatment was significantly lower than the control group,the differences were statistically significant(P〈0.05).Conclusion:Asthmatic patients treated with low-dose azithromycin combined with salmeterol xinafoate and fluticasone propionate powder have better therapeutic effects,it is worthy of clinical promotion.
