弹性髓内钉内固定治疗儿童股骨干骨折临床研究

目的：探讨弹性髓内钉内固定治疗儿童股骨干的临床疗效和优势。方法：回顾性分析2007年1月-2012年12月行弹性髓内钉内固定的21例5岁以上儿童股骨干骨折的临床资料及疗效。结果：本组21例均获随访，随访时间6~36个月，平均12个月。术后6周开始逐步进行负重训练，骨折均愈合，愈合时间8~24周。患肢髋、膝关节活动度正常，功能恢复良好，无跛行。出现皮肤激惹症状5例，无髓内钉断裂、弯曲发生；无再骨折及骨髓炎发生。弹性髓内钉术后8周所有骨折达到影像学愈合。平均术后22周取出弹性钉。结论：弹性髓内钉治疗5岁以上儿童股骨干骨折疗效明显，具有手术时间短、创伤小、愈后快以及并发症少等优点，是一种5岁以上儿童股骨干骨折较为理想的临床治疗方法。

Clinical Study of Elastic Intramedullary Nail in Treatment of Femoral Shaft Fracture in Children

Objective：To evaluate the clinical effect and advantages of elastic intramedullary nail in treatment of femoral shaft in children. Method：Retrospectively analyzed the clinical data and curative effect from January 2007 to December 2012,21 cases of children femoral shaft fractures were treated by elastic intramedullary nail. Result：21 patients were followed up,the follow-up time was 6-36 months,mean 12 months;all patients started weight training after 6 weeks operation,All of the fractures healed ,the healing time about 8 weeks to 24 weeks,the children of the hip,knee joint returned to normal activity. The fracture side limb recovered good functional. 5 patients had skin irritation symptoms,without elastic intramedullary nail breakage and bending;no refracture and osteomyelitis. All fractures achieved radiographic union after 8 weeks operation,average elastic intramedullary nails were removed 22 weeks after operation. Conclusion：The elastic intramedullary nail in treatment of femoral shaft fracture in children has obvious curative effect,can shorten operation time,less trauma,healing and fewer complications.
