直肠癌根治术后应用带蒂大网膜骶前填塞

本文报告了我院自1984年10月～1986年5月对20例直肠癌经腹会阴联合切除术后,施行带血管蒂大网膜填塞骶前,会阴创口一期缝合。20例中,男14例,女6例,按Dukes分级,B级11例,C级9例。术后16例(80％)取得了良好效果。术后住院天数最短12天,平均20天。我们于直肠癌切除术后,切开腹主动脉前腹膜达横结肠系膜根部,彻底清扫腹主动脉前脂肪淋巴组织,而后,将带血管蒂大网膜沿腹主动脉前引入骶前,会阴刀口一期缝合。

PRESACRAL OMENTUM WITH VASCULAR PEDICLE TAMPONAGE AFTER RADICAL RESECTION FOR CANCER OF RECTUM

The Present paper reports 20 cases of rectal Carcinoma from Octobcr 1984 to May 1986, which underwent abdominal Penneal resection with omentum of vascular pedicle tamponade prcSaCral. Among them were 14 males and 6 females. According to Dukes classification, B staging and C staging were 11, 9 respectively. 16 cases obtained satisfactory results. The hospital duration averaged 20 days, with the shortest only 12 days. After radical resection for cancer of rectum the retroperitoneum was oppened. This incision approached the mesentery roal of transverse part of colon. The lymph nodes of Pcriaorta abdominalis were widely removed. Then, the pedicle omcntum received its blood supply from the right gasfro-epiploic vessels and was drawn down exlraperitoneally fronl of the aorta abdominalis to reach the pelvic wound. The presacral space was filled with omentum of vascular pedicle and the perineal wound was sutured.
