心电图检查对老年胸痛患者鉴别诊断价值研究

目的探讨心电图检查对老年胸痛患者鉴别诊断的临床价值。方法以我院2010年1月至2011年12月门诊或住院初步诊断为心源性胸痛的老年胸痛患者204例为研究对象,行常规十二导联同步心电图检查,以临床诊断为金标准,统计分析不同部位疾病在异常心电图中的检出率。结果本组204例临床诊断肺及气道疾病49例,心血管疾病76例,胸壁疾病13例,周围神经疾病14例,胆道疾病7例,健康或不明原因45例;心电图检查正常心电图62例,不正常心电图142例;不正常心电图中心血管疾病异常率最高,占97.4%,周围神经疾病异常率最低,占7.1%。结论老年胸痛病因复杂,心源性胸痛在心电图检查中具有较高的异常率,但仍存在特异性较低的缺陷,应引起重视。

The Value of the ECG Differential Diagnosis of Elderly Patients with Chest Pain

Objective To investigate the value of the ECG differential diagnosis of elderly patients with chest pain.Methods 204 cases outpatient or inpatient elderly patients with the initial diagnosis cardiac chest pain in our hospital from January 2010 to December 2011,were checked by a conventional 12-lead synchronous ECG,taked clinical diagnosis as the gold standard,statistical analysis of different parts of the disease in the detection rate of abnormal ECG.Results In this 204 cases,49 cases is lung and airway diseases,76 cases is cardiovascular disease,13 cases wall disease in,14 cases is peripheral nerve disease,7 cases is biliary tract disease and 45 cases is health or unknown causes;62 cases showed normal ECG,142 cases showed unnormal ECG;cardiovascular disease abnormal rate is highest in the unnormal ECG cases,accounting for 97.4%.Conclusion ECG has a higher exception in elderly patients with cardiac chest pain,but there are still low specificity of defects,attention should be paid.
