基于临床路径的五病种住院费用分析

目的了解乌鲁木齐市某三甲医院五种实行临床路径的病种住院费用的影响因素,探索其医疗费用的控制措施,为医疗机构和政府相关部门提供参考。方法对2009年-2010年该院已实行临床路径的五种病种住院费用进行多元回归统计学分析。结果慢性阻塞性肺疾病住院费用的影响因素包含住院天数、药品费比例、入院时情况、出院转归、检查费比例和性别,P〈0.05;腹股沟疝费用的影响因素有年龄、药品费比例、手术方式、手术费比例和住院天数,P〈0.05;自然临产阴道分娩费用的影响因素有住院天数、药品费比例、入院时情况与产程,P〈0.05;计划性剖宫产费用的影响因素有住院天数、药品费比例与入院时情况,P〈0.05;老年性白内障费用的影响因素有住院天数、材料种类、材料费比例、付款方式与性别,P〈0.05。结论影响病种住院费用的因素很多,而同一病种不同病例间存在很大差异,制定病种限价标准时应充分考虑到每个病种的疾病分型以及其他对费用产生影响的因素。
