脂肪栓塞综合征临床早期诊断和治疗

目的 :讨论脂肪栓塞综合征 (FatEmbolismSyndromFES)早期诊断和治疗。方法 :自 1998～ 1999年 ,收治 5例脂肪栓塞综合征 ,以呼吸系统和神经系统症状为主要表现。胸片显示典型“暴风雪”样改变。治疗以保持呼吸道通畅 ,及时大剂量糖皮质激素等对症治疗为主。结果 :5例病人经过一周治疗病情控制 ,两肺病灶吸收 ,约 2周恢复正常。结论 :骨折的病人 ,如出现无明原因的呼吸系统症状或 /和神经症状 ,应考虑合并脂肪栓塞综合征的发生。早期及时大剂量应用糖皮质激素非常重要

Early Clinical Diagnosis and Treatment of Fat Embolism Syndrome

Objective To discuss the diagnosis and treatment of fat embolism syndrome(FES).Methods 5 patients with FES were diagnosed and treated,admitted into our hospital from 1998 to 1999.Of the 5 patients,3 cases were clinically found expression of mainly respiratory symptoms,2 cases,mainly nervious symptoms,1 case skin bleding.Taking a X-ray photograph of the plain chest film pome patients shown a typical change of “snow stormoid”.The patients had been treated in time with large dose of glucocorticoids,in addition to preserve the fluent respiratory tract.Results After 1 week of treatment,the patients conditions were controlled and the lung lession were absorbed,and then the patients were recovered about two weeks.Conclusion The fracture patients occurring the symptoms of respiratory and/or central nervous system,without a cause originated in itself,should be consider having complication of FES.It is very important that the patients were treated in time with large dose of glucocorticoids treat.
