脑出血患者早期死亡的相关因素分析

目的：探讨脑出血（ICH）患者早期死亡的危险因素。方法：对2001-2003年在我院住院的196例脑出血患者临床资料进行回顾性的分析，对17项起病3d内的相关指标进行单因素及多因素Logistic回归分析。结果：Logistic回归分析显示，意识障碍、白细胞计数、年龄、尿素氮、血糖、发热、冠心痛史7个变量进入回归方程比数比（OR），占95％可信区问（95％CI）依次为2．53（1．73～3．71）、1．29（1．14～1．46）、1．09（1．04～1．15）、1．22（1．07～1．40）、1．22（1．05～1．41）、4．50（1．64～12．40）和8．64（1．52～49．21）。回归模型对本组病人分类的总准确率为87．8％。结论：意识障碍、白细胞计数增加、发热、高龄、尿素氮升高、高血糖、合并冠心痛可能为脑出血早期死亡的危险因素。

Risk Factors for Early Death in Patients with Intracerebral Hemorrhage

Objective:To investigate the risk factors of early death for intracerebral hemorrhage (ICH). Methods:Initial datas of 1% consecutive hospitalized patients with acute intracerebral hemorrhage were reviewed. Univariate and multiple logistic regression were performed to determine risk factors for early death. Results: Among all 17 variables involved in analysis, seven factors were finally found out to be significant independent predictors of early death. They were initial disturbance of consciousness ( odds ratio [OR],2. 53;95% confidence interval [95%CI],1.73-3.71),count of leukocyte (OR,1.29;95%CI,1. 14-1.46), age (OR,1.09; 95%CI,1.04-1.15)blood urea nitrogen(OR,1.22;95%CI,1.07-1.40),blood glucose (OR.1. 22;95%CI,1.05-1.41), fever (OR,4.50,95%CI, 1.64-12.40),history of coronary heart disease (OR,8.64;95%CI,1. 52-49.20). The overall correct percentage of the prognostic model was 87.8%. Conclusion:Initial disturbance of consciousness,elevated leukocyte,old age, blood urea nitrogen, hyperglycaemia, fever and coronary heart disease were the risk factors of early death for hospitalized acute ICH.
