计算机辅助设计截骨导航模板治疗肘内翻畸形的初步应用

摘    要:目的 利用计算机辅助设计为肘内翻畸形矫正术手术方案的制定和实施提供一种精确截骨的方法.方法 对2008年7月至2009年4月收治的9例肘内翻畸形患儿进行肘关节连续螺旋CT扫描,将原始Dicom格式数据导人Mimics 10.1软件,三维重建肘关节模型,以.Stl格式保存,导人Imageware 10.0软件,根据内翻情况设计最佳截骨角度,然后建立与截骨范围解剖学形态一致的模板,将模板通过激光快速成形技术生产出实物模板,手术时将建立的截骨导航模板与肱骨截骨部相吻合,沿截骨导航模板进行截骨,钢板及螺丝钉固定.结果 9例患儿截骨角度满意,术后经3~6个月随访,肘关节恢复满意.结论 通过计算机辅助技术设计的截骨导航模板可以为青少年肘内翻畸形矫正术提供一种个性化、精确截骨的方法.

Abstract:Objective To provide an accurate osteotomy method in the treatment of cubitus varus deformity in teenager.Methods Between July 2008 and April 2009.the computer simulation technique was applied to 9 cubitus varus deformity in 9 patients, 6 male, 3 female.The average age was 15 years (12-18 years).All patients underwent CT scan for the three-dimensional reconstruction.The 3D reconstruction models for the desired elbow were generated by using the Mimics 10.1 software.The 3D models of elbow were imported into Imageware 10.0 software.The accurate angle of osteotomy was defined using Reverse engineering, and osteotomy navigation templates were designed according to the anatomic features of the range of osteotomy.These navigational template were manufactured using the rapid prototyping technique.Results Accurate angle of osteotomy was confirmed with postoperative X-ray.After three to six months of follow up, the range-of-motion were satisfactory.Conclusions This study shows a patient-specific template technique that is easy to use and can simplify the surgery.
