99mTe-DMSA用于诊断肾发育不良伴输尿管开口异位

摘    要:目的 通过运用影像学手段诊断肾发育不良伴输尿管异位开口的经验,探讨这一疾病的最佳诊断方法。方法 回顾性分析14例肾发育不良伴输尿管开口异位的病史资料,比较各类影像学检查对异位肾诊断之优劣。结果 14例患儿异位开口位于阴道12例,膀胱颈2例。常用的影像学诊断方法包括:B超发现异位肾的阳性率为35.7%(5/14)、IVP18.2%(2/11)、CT0%(0/2)、MR100%(2/2)、而^99mTe—DMSA为100%(9/9)。结论 肾组织对放射性核素^99mTe-DMSA极为敏感,通过该检查能发现绝大部分发育不良的肾组织。对本病的诊断有重要意义。

Abstract:Objective Hypoplastic kidney with single ectopic ureter was quite rare.Generally,the routine image study can not identify the malpositioned hypoplastic kidney.Misdiagnosis is not uncommon.Clinical experience of this disease for the recent 11years in our hospital were analyzed to find out a convenient and reliable method for the diagnosis of this disease.Methods From1990to2000,14patients were treated in our hospital for hypoplastic kidney with single ectopic ureter.The rate of detecting hypoplastic kidney in different methods was compared.Results Different image study had a different positive detecting rate:Ultrasound was37.5%(5/14),IVU18.2%(2/11),CT0%(0/2),MR100%(2/2)respectively and DMSA was100%(9/9).Conclusions 99m Te-DMSA is a reliable way to detect renal tissue,it is very helpful for the diagnosis of this condition.For those dribbling of urine with normal voiding,if ultrasound can find only one kidney,a routine 99m Te -DMSA examination should be carried out,in order to rule out a possible hypoplastic kidney.[
