摘    要:目的探讨去整合素金属蛋白酶9(A disintegrin and metallo-proteinase9,ADAM9)及血管内皮生长因子蛋白(vascular endothelial growth factor,VEGF)在眼部恶性黑色素瘤组织中的表达及其意义。方法应用免疫组织化学SP法检测49例眼部恶性黑色素瘤和9例正常眼组织中ADAM9与VEGF蛋白的表达,并观察ADAM9和VEGF蛋白表达的相关性。结果 ADAM9蛋白在眼部恶性黑色素瘤中的阳性表达率为77.6%(38/49),而在正常脉络膜组织中完全没有表达(0/9),组间比较差异有统计学意义(P=0.000);VEGF蛋白在眼部恶性黑色素瘤中的阳性表达率为73.5%(36/49),而在正常脉络膜组织中完全没有表达(0/9),组间比较差异有统计学意义(P=0.000)。ADAM9和VEGF蛋白表达均与眼部恶性黑色素瘤组织是否侵犯巩膜密切相关(均为P<0.05),而在不同性别、年龄、肿瘤大小及最大基底径间差异均无统计学意义(均为P>0.05)。相关性分析表明,ADAM9和VEGF蛋白在眼部恶性黑色素瘤中的表达呈显著正相关(r=0.341,P<0.05)。结论 ADAM9和VEGF蛋白表达与眼部恶性黑色素瘤的发生、发展和转移密切相关,联合检测ADAM9和VEGF蛋白有望成为眼部恶性黑色素瘤早期诊断和判断预后的分子指标之一。

Abstract:Objective To explore the expressions and its significance of A disintegrin and metalloproteinase 9(ADAM9) and vascular endothelial growth factor(VEGF) proteins in ocular malignant melanomas.Methods Immunohistochemistry assay was used to detect the expressions of ADAM9 and VEGF proteins in 49 cases with ocular malignant melanomas and 9 cases with normal eye tissue,and the related factors of ADAM9 and VEGF expression were observed.Results The expression rate of ADAM9 protein in ocular malignant melanomas was 77.6%(38 /49),while complete negative expression of ADAM9 was found in the normal eye tissues(0 /9),there was statistical difference between two groups(P = 0.000).The expression rate of VEGF protein in malignant melanomas of eye was 73.5%(36 /49),while complete negative expression of VEGF was demonstrated in normal eye tissues(0 /9),there was statistical difference between two groups(P = 0.000).Additionally,the expressions of ADAM9 and VEGF proteins were closely correlated with the tumor invasion into the sclera(all P < 0.05),but not correlated with gender,tumor size and most basal diameter(all P > 0.05).Further,the result of correlation analysis revealed that expressions of ADAM9 and VEGF proteins in malignant melanomas of eye displayed positive correlation(r = 0.341,P < 0.05).Conclusion ADAM9 and VEGF protein play important roles in the infiltration,metastasis and carcinogenesis of ocular malignant melanomas eye.United examining ADAM9 and VEGF protein become one of the molecular diagnosis and prognostic factors in the early ocular malignant melanomas.
