LASEK和Epi-LASIK术中去除上皮瓣治疗中低度近视疗效对比

目的比较准分子激光上皮瓣下角膜磨镶术(LASEK)和机械法准分子激光上皮瓣下角膜磨镶术(Epi-LASIK)术中去除上皮瓣治疗中低度近视的临床效果。方法将78例患者(-1.50～-6.00D)152眼随机分为A组和B组:A组行去上皮瓣的LASEK,B组行去上皮瓣Epi-LASIK。观察术后眼球疼痛刺激症状、角膜上皮愈合情况,比较2组术后1个月、3个月、6个月裸眼视力及术后6个月haze情况。结果 A、B两组术后3d眼球疼痛刺激症状0、1、2、3级例数比较,差异均无统计学意义(χ2=3.69、0.00、0.00,均为P>0.05)。A组角膜上皮平均愈合时间为(3.81±0.07)d,B组平均为(4.15±0.12)d,2组比较差异有统计学意义(t=2.42,P<0.05)。术后1个月、3个月、6个月2组裸眼视力和术后6个月haze比较差异也均无统计学意义(均为P>0.05)。结论 LASEK去除上皮瓣矫正中低度近视与Epi-LASIK去除上皮瓣矫正中低度近视相比,角膜上皮愈合更快,而术后视力恢复和haze的形成无明显差异,更具有临床应用价值。

Clinical efficacy comparison of LASEK and Epi-LASIK(with flap free) for moderate and low myopia

Objective To compare the clinical efficacy of LASEK and Epi-LASIK with flap free for low and moderate myopia.Methods Seventy-eight myopic patients(152 eyes,from-1.50 D to-6.75 D) were divided randomly into group A and B.LASEK with flap free was performed in group A,Epi-LASIK with flap free in group B.All subjective ocular pain scores and corneal epithelial healing were observed.Uncorrected visual acuity at postoperative 1 month,3 months,6 months were compared.Postoperative haze scores were recorded.Results There was no statistical difference in eyes that subjective ocular pain scores at grade 0,1,2,3 at postoperative 3 days between two groups(χ2=3.69,0.00,0.00,all P>0.05).The average time of corneal epithelial healing in group A and B were(3.81±0.07)days,(4.15±0.12)days,there was statistical difference between two groups(t=2.42,P<0.05).There was no statistical difference in uncorrected visual acuity at postoperative 1 month,3 months,6 months and haze scores at postoperative 6 months between two groups(all P>0.05).Conclusion The time of corneal epithelial healing in LASEK with flap free is shorter than that in Epi-LASIK with flap free,no difference in postoperative visual acuity recovery and haze formation between them is found,so LASEK with flap free has better clinical application value for moderate and low myopia.
