囊袋张力环在高度近视眼后发性白内障手术中的应用

目的探讨囊袋张力环(capsular tension ring,CTR)在高度近视眼后发性白内障手术中的应用效果。方法选择2009年6月至2011年12月我院高度近视眼后发性白内障患者320例(545眼),均为晶状体屈光术后患者,随机分为对照组156例(270眼)和观察组164例(275眼)。对照组采用常规超声乳化白内障摘出联合人工晶状体植入术,观察组在对照组手术基础上植入CTR。术后随访18个月,观察两组患者术中、术后并发症的发生情况,同时记录末次随访时视力、眼压的变化以及白内障的复发情况。结果对照组术前视力为0.23±0.09,术后为0.37±0.10;观察组术前视力为0.25±0.08,术后为0.45±0.09;2组患者术后视力均较术前提高,与术前比较,差异均有统计学意义(均为P<0.05);2组术后视力比较,观察组好于对照组,差异有统计学意义(P<0.05)。2组患者术后眼压均较术前降低,与术前比较,差异均有统计学意义(均为P<0.05);2组术后眼压比较,观察组低于对照组,差异有统计学意义(P<0.05)。2组白内障复发率、并发症发生率差异均有统计学意义(均为P<0.05)。结论 CTR作为新型的白内障手术辅助材料,能够有效地提高高度近视眼后发性白内障手术的治疗效果。

Clinical application of capsular tension ring in high myopia patients with after-cataract

Objective To discuss the clinical surgical effects of capsular tension ring (CTR) in high myopia patients with after-cataract. Methods A total of 320 cases (545 eyes) with after-cataract in high myopia in our hospital from June 2009 to December 2011 were chosen,all cases were previously performed the refractive surgery.The patients were randomly divided into control group (156 cases,270 eyes) and observed group (164 cases,275 eyes).The patients in control group were treated with phacoemulsification and intraocular lens implantation,and the patients in observed group were treated with capsular tension ring in addition.The follow-up time was 18 months,the intraoperative and postoperative complications in two groups were observed,and the visual acuity and intraocular pressure at the final follow-up were recorded,the recurrence of cataract was also observed. Results The preoperative and postoperative visual acuity in control group were 0.23±0.09 and 0.37±0.10,which in observed group were 0.25±0.08 and 0.45±0.09,the postoperative visual acuity in two groups were improved,compared with pre-operation,there were statistical differences (both P<0.05);The postoperative visual acuity in observed group was better than that in control group,there was statistical difference(P<0.05).The postoperative intraocular pressure in two groups were all decreased,compared with pre-operation,there were statistical differences(both P<0.05);The postoperative intraocular pressure in observed group was lower than that in control group,there was statistical difference(P<0.05).There were statistical differences in recurrence of cataract and complication rates between two groups(both P<0.05). Conclusion As a new type of cataract surgery supporting material,CTR can effectively improve the clinical surgical effects in high myopia patients with after-cataract.
