LASIK术后前房变浅及其相关因素分析

目的探讨近视眼准分子激光原位角膜磨镶术(LASIK)后中央前房深度改变及其相关影响因素。方法采用A超、IOL-master、Orbscan及Pentacam四种不同仪器设备测量中央前房深度,IOL-master测量眼轴长度,A超测量晶状体厚度,统计分析其术前、术后1周、1个月、3个月及6个月的变化情况。结果 LASIK术后1周、1个月、3个月及6个月测量结果与术前相比:A超、IOL-master、Orbscan及Pentacam四种不同仪器设备测量的中央前房深度较术前均有明显下降(P≤0.009),A超测量的晶状体厚度较术前均有明显增加(P=0.000)。LASIK术后6个月前房变浅与年龄、术前近视度数、角膜曲率、中央角膜厚度、切削深度、眼轴长度变化无关(均为P>0.05),与晶状体厚度增加呈负相关(P≤0.002)。Orbscan测量的LASIK术后6个月前房变浅与准分子激光切削区大小呈正相关(r=0.247,P=0.046)。结论 LASIK术后中央前房变浅与晶状体厚度增加有关,与检查设备成像系统的放大效应无关。LASIK改变了Orbscan在测量前房深度时的成像系统放大效应。

Analysis of anterior chamber depth reduction and its correlative factors after LASIK

Objective To discuss the central anterior chamber depth(ACD) changes and its correlative factors after LASIK.Methods The central ACD was measured by Pentacam,Orbscan,IOL-master and A-scan ultrasound,the ocular axis length by IOL-master,and the lens thickness by A-scan ultrasound,the changes of them between preoperative and postoperative 1 week,1 month,3 months and 6 months were statistical analyzed.Results Compared that before operation,the central ACD at postoperative 1 week,1 month,3 months and 6 months were decreased significantly(P≤0.009),and lens thickness was increased significantly(P=0.000).The ACD reduction at postoperative 6 months was not correlated with age,preoperative refraction,corneal curvature,central corneal thickness,ablation depth and ocular axis length(all P>0.05),but negative correlated with lens thickness increasing(P≤0.002),and positive correlated with the optical zone diameter of artherectomy(r=0.247,P=0.046).Conclusion The central ACD reduction after LASIK is correlated with the lens thickness increasing,and not correlated with the magnification effect of examination system.ACD measured with Orbscan is affected by the changes of the magnification effect of the post-LASIK corneas.
